Case 1/2008: three year old female patient with subvalvular, valvular, and supravalvular pulmonary stenosis.

Dados clínicos Paciente do sexo feminino, com sopro cardíaco auscultado de rotina aos 12 meses de idade, mantendo-se sem sintomas desde o nascimento e sem intercorrências clínicas. Ao exame físico, a paciente apresentou-se eupnéica, corada, e com pulsos normalmente palpados nos quatro membros. Seu peso era de 15 kg; altura, 100 cm; pressão arterial, 95/60 mmHg; e freqüência cardíaca, 86 bpm. A aorta não era palpada na fúrcula. O precórdio apresentava-se sem deformidades e impulsões, e o ictus cordis não era palpado. A paciente apresentava frêmito sistólico discreto na fúrcula e borda esternal alta, com sopro sistólico +/++, em ejeção, rude, nos mesmos locais. As bulhas cardíacas eram normofonéticas, estando a segunda bulha constantemente desdobrada, com o componente pulmonar menor que o aórtico. Pulmões e abdome sem anormalidades. O eletrocardiograma demonstrou ritmo sinusal e sinais de sobrecarga das cavidades direitas. ÂP: +60o; ÂQRS: indeterminado; ÂT: +30o.
